The documents needed for registration are: an original birth certificate with a raised seal, immunization records (including three polio vaccinations, three DPT vaccinations, MMR (including a second measles vaccination given after 15 months of age), three Hepatitis B vaccines, and the Varicella vaccination) signed by the physician or clinic, and three (3) forms of proof of residency in North Babylon; one form of proof should be the 2005-06 or 2006-07 Town of Babylon Property Tax Bill. For renters one form of proof should be a notarized Landlord's Affidavit; this form may be picked up at any elementary school or Room 105 (District Registrar 's Office) at North Babylon High School.
