Diana H. Formas (nee Kipybida), of West Islip, died July 14, 2002 at the age of 71.

Mrs. Formas is predeceased by her husband, Skip. Surviving her are her children, Jayne and Harry Manko of Bay Shore; her grandchildren, Tim, Melissa and Billy

Mrs. Formas reposed at the Fredrick J. Chapey & Sons Funeral Home, 1225 Montauk Hwy, West Islip. A funeral mass was said July 18, 2002 at Holy Family Ukrainian Church, West Islip. Interment was in Pinelawn Memorial Park.
